Was it the CISO’s fault? Did they accept risk for a know. issue and take no action? Did they choose not to implement end user security training? Did they ignore poor end user security hygiene? Was it within their power and control to prevent? I answered no, but this is not a simple yes/no question. It’s extremely situational.

I don't think this is a yes/no question. Like most answers to information security questions, answer is 'it depends'.
