The preference is JIT but that is not an easy option for on-prem systems, so we reserve it for the most privileged roles in the cloud.  Enhanced MFA is also in place.

We’re re-evaluating our IAM and MFA strategy across a large academic health system. A recent proposal recommended merging all AD domains and applying blanket MFA (DUO) for every user.
