Does your company provide employees with Cell Phone and Internet Reimbursement regardless if they work from home office or company office? If so how much?

I've experienced and deployed different arrangements and so with pros and cons for each one of them. Let me outline them here.
1. Company provided mobile and company reimbursed Internet
# company provided mobile phone as per choice of the employee but only above certain job grade (senior and executive positions)
# some employers (in my own case the global US companies who I used to work for) covered all cost irrelevant whether business or personal calls
# my most recent two employers in the Middle East expected for personal calls to be declared by the employee and these charges got distracted from monthly payroll
# company reimbursed for home Internet connection for all employees who could justify a reimbursement request with a reasonable business reason - whether "work from home" or not
# company reimbursed for home Internet connection only for employees who had to be mobile and flexible as per their role as well as for senior and executive positions
2. Company provided SIM card and company reimbursed Internet
# same like above but instead of company providing a phone plus SIM card, only SIM card has been provided above certain job grade (senior and executive positions)
# Internet reimbursement - same scenarios like above
3. Company provides no phone, no SIM card but reimburses Internet
# company provides for fixed amount mobile phone allowance for all employees who require mobile phone in line with their role and responsibility

4. Company provides no phone, no SIM card but reimburses for business calls and reimburses Internet
# company only accepts mobile usage reimbursement by employees with business need and for business related calls
# Internet reimbursement - same scenarios like above
5. Company caters for one of the above mentioned mobile phone scenarios but no Internet reimbursement
# company caters for one flavor of the above mentioned mobile phone scenarios but Internet isn't reimbursed at all as it's assumed that people have a home Internet connection anyway

Yes, the company provides up to $50 per month for internet and $100 per month for phone charges. In some cases, company provides the SIM (telephone connection) and the charges are directly borne by the corporate.
We reimburse cell phone plan (not device) for employees that are expected to be more available, like management and executive assistants. Standard is $50/mo and for those that need to be highly available it's $70/mo.
