Very likely a technical mistake made with good intentions. However if that is the case the timing was poor unless on a global view that change was made at the lowest typical use time

Just glad I didn’t approve that Change Control Ticket!!

I'm going quite broad and saying human error. Someone, somewhere, has made a change that wasn't fully tested.

By timing, are you referring to how quickly it comes after the Frances Haugen interview?
