How does your organization calculate the promotion rates, particularly in the context of promotions vs. headcount or promotions vs. eligible population? Wondering what formula everyone else is using and why.

I use Promotions / Average Headcount. We do not have clearly defined policies for determining when someone is eligible for a promotion, making the second metric difficult/impossible to calculate.
