Has anybody removed the "prefer not to say" option when asking for equality data?  If so, did this cause any issues?

No, when gathering statistics on equality, our organizations did not stop offering the "prefer not to say" option. Removing the choice, however, should, in my opinion, allow organizations to focus on improving the completeness and accuracy of their data, which will enable them to more effectively identify and address discrepancies.
