It would help if there was a definition of incident.  Is it "alert that requires response (but didn't necessarily cause impact)?"  Is it "alert that caused impact?"

Yes, this is a key point. There’s something between “we did a lot of things in the course of business to address something” vs “we had an unplanned incident response” vs “we had something they caused impact.” Different places definitely define these things differently.
