It's been more than 6 or 7 years since we've started working with Agile practices.
It works great with some projects and not so great for others depending on the nature of the job to be accomplished.
The biggest error: having people thinking that it's a One-Size fits All kind of approach. It's not.
Sometimes, you can't create value iteratively and you can't take decisions during the whole project. It has to be done in a more waterfall or traditional way.
This is specifically true for very large programs (that encompass multiple projects).
