Here's what I would use in a survey to assess stakeholder satisfaction after an IT implementation project (and I'm assuming your customers = stakeholders in this):
On a scale of 1 to 10, how satisfied are you with the overall outcome of the IT implementation project?
Were the project goals and objectives clearly communicated to you throughout the process?
How would you rate the communication and collaboration between the project team and stakeholders during the implementation?
Did the IT implementation meet your expectations in terms of functionality, usability, and performance?
Were your concerns and feedback adequately addressed during the project, and do you feel your input was valued?
If you are bold you can start collecting NPS score and ask on a scale 1 - 10 how do you rate working with me/us (the team) and an option field WHY?
These questions cover overall satisfaction, communication effectiveness, alignment with expectations, and the stakeholders' perception of their involvement in the project.
