Which technology do you use for your diversity questionnaire?  How do you collect information regarding the make up of your employee population (asking about gender, race, ethnicity, disabilities, etc.)? Most HRIS/HCM offer this functionality natively.  Our organization is made up of multiple affiliates with their own HRIS.  We are looking for an unified diversity questionnaire across our core corporation as well as our affiliates outside our main HRIS. Ideally, the diversity questionnaire would have a great UX to drive high adoption.  Although many diversity factors are static in time (e.g. race), we want employees the ability to revise their diversity questionnaire at any time (e.g. you now have a disability).

We use an advanced version of Survey Monkey-we can also use Glint in pulse surveys if we coordinate with Global first.  Glint is used for global engagement surveys.

We are using our HRIS system - workday. Employees have the ability to update their information at any time.
