I do know of someone who did, but it is rare and they didn't stay long.  Seems most people who I know are successful consultants enjoy the constant change of where they are working.  Long term successful CIO's seem to stay because they enjoy staying with people they know and helping them be successful in the long term.
CIO, Self-employed
I have moved from CIO to consultant back to CIO. Frankly, consulting job is too easy if you are working for one of the big consulting companies as there is no operational pressure. I personally think cio job is much tougher and more accountable and you are on your toes 24x7

I don't know of any, but that doesn't mean it has never happened. I think the path would involve first moving from consulting to full time permanent work at the organization in question, then working up the chain, so it's not going to happen overnight.

I have gone back and forth between being a CISO and a Consultant. It is hard for me to stay focused and sitting at a desk 8-10 hours a day 5 days a week on site in an office. I much prefer clients shifts and being more of a vCISO and in the consultant world. I have found that a lot in the industry that are consultants are ADD and need the shift in environments. I am hyper ADD and love the CISO role but enjoy the "chaos" of the vCISO world and constant shifting and juggling different client's ongoing initiatives.

By far the best place for me to travel was Shanghai. Loved the city and the vibe. Singapore is also an amazing place to have to be stationed for work.
