It examines the methodologies members use to conduct a root cause analysis and their approaches for capturing recommendations, actions, and follow-ups to decrease the risk of recurrence.
How do members ensure that they are executing root cause analysis in standard ways and consistently capturing information, recommendations, and follow-up actions to minimize the risk that an issue will recur?
All 13 respondents perform root cause analysis for “major,” “high severity,” or “priority 1” incidents.
Slightly more than half of the respondents (7 of 13) use ServiceNow to track incidents and decrease the risk of recurrence.
Almost all respondents (11 of 12) create a problem record as the first step in conducting root cause analysis.
